The understanding of nanomaterial toxicity is aided by biokinetic information pointing to potential target organs. Silver (Ag), copper oxide (CuO), and zinc oxide (ZnO) are often referred to as soluble materials in the literature. In addition, data suggest gold (Au) nanoparticles to be soluble in the mammalian body. We identified inhalation studies on these materials and extracted data on physicochemical properties, organ distribution, and excretion. Silver and gold were retained in the lung for an extended period (>2,000 and >672 hours, respectively); copper initially increased in lung and then returned to baseline at ∼500 hours. Zinc increased in the lungs after short-term exposure to zinc oxide, but not after prolonged exposure. In blood, silver initially increased after inhalation but then gradually declined over ∼200 hours. Gold was elevated in the blood after exposure to 4, 7, 11, and 13 nm particles (but not particles of 20, 34, and 105 nm) and remained elevated for at least 672 hours after exposure to the 4 and 11 nm particles. Silver increased in the liver and spleen and was still present 2,000 hours post exposure. Gold was elevated in several organs, including the spleen and kidney, for more than 600 hours post exposure, indicating persistence in some organs. Both silver and gold were increased in the brain and olfactory bulb. Overall, we found no large differences in the biodistribution of the four nanomaterials but note that silver and gold were still increased in several organs at the last investigated post-exposure time points.

2. Evaluation of the Efficacy of a Serum Containing Niacinamide, Tranexamic Acid, Vitamin C, and Hydroxy Acid Compared to 4% Hydroquinone in the Management of Melasma.

In a randomized, investigator-blind trial (n=60), a niacinamide–tranexamic acid–vitamin C–hydroxy acids serum achieved comparable melasma improvement to 4% hydroquinone at 3–5 months. The serum showed better tolerability (less erythema) and superior skin hydration/barrier metrics, with significant QoL gains in both groups.

INTRODUCTION: Melasma is a common skin condition that remains challenging to treat. Hydroquinone at 4% (HQ4%) is a frequently prescribed depigmenting compound that has been associated with potential side effects. OBJECTIVE: This study assessed the benefit in melasma of an anti-hyperpigmentation serum (Serum B3 containing 5% niacinamide, 1% tranexamic acid, 0.2% of a stabilized form of vitamin C, and different hydroxy acids) compared to HQ4%. MATERIALS AND METHODS: In a single-site, investigator-blind, randomized study, 60 females aged between 20 and 50 years with facial melasma received Serum B3 for 5 months (Group 1) or HQ4% for 3 months followed by Serum B3 for an additional2 months (Group 2). Endpoints were Melasma Area and Severity Index (MASI), modified MASI (mMASI), Investigational Global Assessment, erythema, clinical cutaneous parameters, and safety. Subjects assessed quality of life (QoL) and cosmetic acceptability. Confocal reflecting microscopy was performed. RESULTS: A significant (p < 0.001) reduction in pigmentation was seen in both groups after 3 months. A higher erythema score was noted in Group 2. Hydration and skin barrier function performed better in Group 1. QoL significantly (p < 0.001) improved in both groups after 84 days. Serum B3 was tolerated better than HQ, and subjects appreciated Serum B3. Melanin density reduction was similar for both groups after 3 and 5 months. CONCLUSION: Serum B3 used for 5 months and HQ4% applied for 3 months, followed by a 2-month use of Serum B3, had a similar efficacy profile, with Serum B3 having a better local tolerance and patient acceptability.

BACKGROUND AND OBJECTIVE: Carboxytherapy (CXT) is a minimally invasive procedure based on percutaneous or subcutaneous microinjections of carbon dioxide. Due to the Bohr effect, it influences the hemoglobin dissociation curve, improving tissue oxygenation, angiogenesis, facilitating vascular perfusion, and lymphatic drainage. Recent evidence suggests unconventional applications in dermatologic surgery and aesthetic medicine; these are focused with the present review. METHODS: A systematic review was conducted by searching the PubMed library from January 2018 to July 2023, using the terms "Dermatologic Surgery, Aging, Rejuvenation, Scars, Cellulite, Alopecia, AND Carboxy therapy." Eligibility criteria included original studies aimed at discussing CXT in dermatologic surgery and aesthetic medicine. RESULTS: The final synthesis included 41 articles distinguished by topic into studies on the role of CXT in facial rejuvenation ( n .14), in cellulite, scars and fat deposits treatment ( n .17), and other pathologic conditions ( n .10). Overall, CXT is an effective antiaging treatment. Furthermore, CXT may play a role in several pathologic conditions including alopecia, lichen planopilaris, vitiligo, chronic diabetic wounds, temporomandibular arthralgia, and lipedema. CONCLUSION: CXT, by promoting tissue oxygenation and microvascular and vascular perfusion, appears to have promising indications for several aesthetic and pathologic conditions, potentially offering satisfactory results with a minimally invasive, safe, and well-tolerated treatment.
